There are variances to pain, including tolerance to heat and cold. The times of day, activity, and even body position play into the hand. When you put the cards down, it is YOUR tolerance which decides how much you can take. These guidelines are just for reference. 
So how do you rate pain? By activity, time of day, and other factors. Then get your average for a day and a few days.
Me? Well, I am at level 3 or 4 daily but won't take medication - I need to stay in control. Worst days, I hit 8. The lesson here? Never put a Econo-van under a tractor trailer rig, the damage is unreal to the body!
